The Great Hormone Debate

In November 2025, the FDA made a big announcement. They will remove the broad “black box” warnings from hormone replacement therapy for menopause. This decision came after a thorough review of studies and expert opinions. Evidence shows that the fears about hormone therapy are mostly unfounded, especially for women who begin treatment at the right time.
The core question is no longer whether hormone therapy is inherently dangerous. It’s about knowing who benefits, when to begin, and how to reduce risks with personalized treatment.
A Key Discovery: “Timing Hypothesis”
Women who start hormone therapy within 10 years of menopause had lower cardiovascular risk. Some studies suggest heart disease risk dropped by up to 50%. The higher risk showed in women who began therapy long after menopause. Women experiencing perimenopause or who are in menopause are great candidates for hormone therapy.
Hormone therapy helps prevent osteoporosis and bone fractures. It can cut fracture risk by 50 to 60%.
Quality of Life
Hormone therapy is the best treatment for hot flashes and night sweats. These can happen many times a day, disrupting sleep, work, and daily activities. New studies show that starting hormone therapy early may help prevent cognitive decline. It could lower the risk of Alzheimer’s disease by as much as 35% if begun during menopause.
Personalization: The New Gold Standard
Modern hormone therapy is all about customization. For women, FDA-approved bioidentical hormones have been well-studied and are generally preferred.
Current practice follows the “lowest effective dose” philosophy. For women, the goal is to use just enough hormone therapy to relieve symptoms and protect bone health.
